IRIS is available in all counties that currently offer Family Care and/or Wisconsin Partnership through the State of Wisconsin’s managed care expansion. As managed care expands to additional counties across the state, IRIS will be available in those counties as well.

If you are already in the Family Care or Wisconsin Partnership program, you may go directly to your local Aging and Disability Resource Center to begin the referral process. If you would like additional assistance, you may let your care manager know that you are interested in joining IRIS. He or she will help you work with the Aging and Disability Resource Center to complete a referral to IRIS.

If you are not enrolled in a Medicaid funded long-term care program, contact your local Aging and Disability Resource Center to find out if you are eligible for IRIS or other long-term care programs.

If you live in county that has not yet begun offering Family Care, contact your local Aging and Disability Resource Center or Human Services Department to find out when IRIS will be available to you. They may begin the referral process up to 6 months prior to IRIS becoming available in your county.

If you are currently on a CIP or COP-W program and are interested in IRIS, your current care manager can make a referral directly to IRIS.
