IRIS is unique among Medical Assistance waiver programs. Providers play an important role as IRIS Participants develop their network of supports and resources. This group is a place to share ideas and information, learn more about processes, and get answers to questions.
Please join our group and contribute. We would like you to share what services your organization offers, what counties you serve, and upcoming events. We ask that providers not include pricing in their posts. IRIS does not enter into agreements (e.g. rates, terms of service) directly with providers.

2012 SDS Network Event - Opening Doors to Community Supported Living. At the SDS Network Event, participants will learn about innovative efforts and share ideas that will help improve long-term care outcomes for individuals with disabilities and their families. I've been a DVR job developer and Supported Employment job coach for over ten years. I'm excited about the IRIS Program for the unique opportunities it provides for opening doors for long-term employment partnerships with community employers. I look forward to sharing with you how we've done this in Vernon County.

We are a technology company that has been providing products and services to help seniors remain comfortably in their homes for as long as possible for the last 20 years. Healthcom provides personal emergency response equipment and services (ask about our portable, cellular, GPS enabled device that does not require a home phone line), medication management devices and monitoring as well as telehealth equipment and monitoring to collect vital signs remotely. Thanks for the positive feedback. The TAP-it is an incredible tool. There are about 800 units in schools and centers across the country. Some of the units in WI are at: the Lakeland School in Elkhorn, Watertown School District, Multiple Milwaukee Public Schools (9), Milton Schools, Powers Elementary in Beloit-Turner, Kettle-Morraine, Cedarburg Early Learning, CESA 2 Wetosha, ...
There's also been a unit on loan to the UCP of Grater Chicago-Infinitec. There's a video on our site with it being used with adults in their program.
I would be glad to do an online demo for anyone in the group or for referrals. The videos on the website (click the button in the upper right corner of the page) are also a great place to see how various schools are using it. 800-251-4077 X362
I would be interested in receiving info on how you see the TAP-it qualifying for the program you work with and/or the educational side of things too. Are there written guidelines or parameters that would be helpful for me to know? Thanks.
