A great opportunity to get first hand expert advice on Community Supported Living is available this summer at the 2 Day Community Supported Living Events in Milwaukee and Appleton.

Trainings will be led by Dennis Harkins, Peter Leidy and Jen Squire. 

Growing Knowledge, Partnership and Self-Determination

 Event Dates and Locations

 Milwaukee Series Tuesdays - June 18 and July 23

OR

Appleton Series  Mondays - June 19 and July 24

*Participants need to commit  to both days of the series.

 The 2 day learning event is designed for individuals, families, service providers, MCOs, IRIS and County Legacy Programs interested in learning how Community Supported Living assists people to live in their own homes.

You will leave the two day series with practical skills that enhance your ability to create Community Supported Living in your life.  

 

Choose the Series Nearest Your Home Community:

This training event is designed in two parts.
Day One: Participants will learn how Community
Supported Living can work in Wisconsin. This is a
practical guide.  Between sessions participants will work on their own before returning for a deeper
exploration.

Day Two: “Next steps” in developing Community
Supported Living in you community.

 
After the training follow-up assistance from InControl Wisconsin will be available to participants after the event.

Opening Doors to Community Supported Living

Wednesday, July 11, 2012

8:30 a.m. – 4:00 p.m.

Kalahari Resort, Wisconsin Dells

 

The Self-Directed Supports (SDS) Network is designed to connect people and ideas to propel self-direction in Wisconsin!

Participants will learn about innovative efforts and share ideas that will help to improve long-term care outcomes for individuals with disabilities and their families.

Be part of the movement as Community Supported Living develops across Wisconsin! Learn more about the exciting work of our guest presenters:

  • Sandy VanEck, Director of Innovation and Design, and Chris Liuzzo, Associate Executive Director, will share the exciting journey of their agency and the individuals and families they support in Rensselaer County, New York.
  • Joe Erpenbeck is the Supervisor of ABCD Cincinnati, Ohio. His team supports people in becoming connected to people and places in their neighborhoods based on their gifts.  
  • Annette Downey is the Executive Director of Community Living Services in Oakland County, Michigan. Annette is committed to supporting people to live in their own homes; to establish meaningful, long-term relationships; and to be active and contributing members of their communities.
  • Pris Boroniec and Beth Wroblewski from the Wisconsin Department of Health Services (DHS), along with other key staff working on Long-Term Care Sustainability Plans.
  • Plus many Wisconsin contributors who are creating community supported living opportunities in Wisconsin.

Agenda

  • Keynote by Sandy VanEck and Chris Liuzzo from Innovation and Design, Rensselaer County, New York: Agency transformation to support people leading lives of distinction in their communities
  • Developing individualized supports to enable people to live in homes of their own
  • Technology to enhance the safety and quality of people’s lives at home
  • Transforming group homes using Community Supported Living strategies
  • Development of Community Supported Living in Family Care and IRIS
  • How Individuals, families and agencies can partner to put dreams, imagination and creativity into practice
  • Lunch presentation from DHS staff on Sustainability Plans. You will have the opportunity to learn more about the plans and provide input

 

Who should attend: People interested in creating Community Supported Living options for individuals with disabilities. This includes: individuals with disabilities, family members, residential and support providers, Managed Care staff, IRIS consultants and DHS department staff.

The IRIS ICA Spring Regional Trainings will include participants sharing their stories - we are looking for the participant experts who have used IRIS to meet their Supported Living Goals.  Is that you ?  If so please see application process below -

Experts In Practice Team Applications are Now Available

The IRIS Regional Meetings this spring are an opportunity for IRIS staff, participants, and families to learn about Integrated Employment and Community Living through the direct experience of IRIS Participants.

IRIS Participants who are interested in sharing their knowledge of Integrated Employment and Community Living can sign up to be part of an Experts in Practice Training Team. These Experts in Practice will be guest presenters at the upcoming IRIS Regional Meetings. There will be a selection process for interested participants.

Barb ... I am not awareof any supported living providers who have gone out of business, and do not believe that has happened. Are there rate issues with MOCs? Certainly, and that is a complex issue. Our challenge, which I believe we can meet, is to show how Supported Living can be a cost-competitive alternative to other kinds of residential services, most of which are continuing and expanding throughout the state.

The work we are doing through inControl on expanding will include a directory of agencies now offerring Supported Living, as well as those interested in expanding to other parts of the state. It will also work with groups (including families) who would like to develop their own agency, and will work with other service providers interested in learning how to do Supported Living.

Thanks for sharing your story with us, Karen, and good luck with the roommate search.

Dennis, you mentioned earlier that people will need secure, stable, life enhancing agencies to carry on the work for supported living to endure. Do we know what our state DHS is doing to support that goal? I have heard of agencies (that had been in business awhile already and who were providing great services) who have been driven out of business by inadequate CMO rates. Preferred providers don't necessarily equal choice and self determination. In sparsely populated areas, if one provider bites the dust, that might be the only provider. Are rates being set with quality, choice, and stability in mind?

Is there a data base of agencies who work with supported living situations statewide? Might some of them join this group and let us know of their experiences?

Laura ... I would contact someone directly at the IRIS Consultant Agency about your caregiver exchange idea. Sounds like a great idea for another part of the IRIS Web Site.
