Bilingual IRIS Consultant

The Employer Group, are seeking a Hmong-speaking IRIS Consultant (IC) in Milwaukee, Marathon and Portage counties to provide IRIS Participants with the assistance necessary to meet their dreams/outcomes and to identify and implement a community-based network of supports, as chosen by the participant. IC's have collaborative relationships with participants, which are based on the level of support the Participant requests of the IC. IC's must be available and must respond to the participant in a timely manner because the Participants rely greatly on the IC to access resources and services. IRIS Consultant Mentor
The Management Group, Inc. and
The Employer Group, acting as co-employers, are seeking an IRIS Consultant Mentor who lives in Dane, Iowa or Sauk county area and will serve the southwestern counties of Wisconsin. This is a work-from-home position. The Consultant Mentor provides leadership, direction and support to a group of IRIS Consultants as they fulfill the needs of the IRIS participants within the parameters of the IRIS program.
TMG is the contracted IRIS Consultant Agency (ICA) for the IRIS program. IRIS is a Wisconsin long-term support program where eligible individuals self-direct their publicly funded, community based long-term care services.
Responsibilities: Establish relationship with each direct report IRIS Consultant (IC) and provide individualized attention and support as appropriate and needed; interpret and communication policies; provide training; review plans and other documentations of all new ICs until work reflects appropriate competencies levels; act as level of support in trouble shooting and de-escalating tenuous situations with participants and their support networks; assess all AA/EE requests to determine completeness and approve for submittal or return to IC for further action/documentation; as needed with ICs and others, assess situations, provide solution and follow up on results in order to maintain protocol and safety for ICs, participants and partners; hold, at minimum, weekly meetings with new solo-ready ICs and monthly meeting with each individual direct report to assess performance and provide support; hold IC mandatory team meetings three times per year to provide program updates, share ideas and issues, and team-building; communicate performance issues and discuss possible solutions with IC Coordinator; keep ICs, other mentors and relevant parties informed of important topics as appropriate; act as change management conduit and support in assessing or interpreting program, policy and protocol changes, staff changes and conflict of interest situations; work collaboratively with other mentors, Orientation Consultants, participants, family guides, ICs, and the ICA, etc. to ensure quality service is being provided to participants; act appropriate and accordingly to tasks delegated by IC Coordinator; run reports to include participant support tracking and forecasting, and act on outstanding needs and issues; act as liaison between ICA and ICs to provide complete information and communication and ensure quality support; help ensure that the correct questions and/or information get to the appropriate person, department or entity in a timely fashion; perform yearly IC field evaluations and provide feedback for improvement and reinforcement to ICs and IC Coordinator; provide performance counseling, with input from IC Coordinator and HR, for ICs as needed; conduct random sample of 20% of ICs each month to review case notes, ISPs, etc., to ensure documentation meets program requirements; communicate any discrepancies to IC Coordinator to determine action items; and other duties as assigned and/or available.
Qualifications: Associate's degree, or commensurate experience, and at least two years experience managing staff within a waiver or similar governmental program or in a human services field extremely desirable; excellent communication skills to include written, oral and listening skills; strong customer service skills and quality focus; conflict resolution skills and ability to act as an advocate as appropriate; good organizational and time management skills; leadership, management/supervision, coaching/mentoring and consulting skills and experience; the ability to work cooperatively as part of a team; the ability to be resourceful, proactive and detail-oriented; the ability to follow policies and procedures as applicable and appropriate at all times; the ability to maintain the strictest of confidentiality at all times; the ability to operate in an open, honest and forthright manner in order to build and maintain relationships; the ability to problem solve, troubleshoot and follow through/persevere as needed and appropriate; the ability to remain calm and rational in high-pressure situations; the ability to work both collaboratively and independently, without close supervision; ability to be both self-motivated and patient; flexibility in the work environment and willingness and ability to adapt to changing organizational needs; must possess a valid driver's license, maintain adequate auto insurance for job-related travel, and ability to travel within Wisconsin and other states as necessary; demonstrated computer and software skills required; and proficiency with Microsoft Office Suite and experience entering and managing data and producing reports within a database.

The Orientation Consultant is responsible for a broad range of planning activities including providing participants with a comprehensive orientation to the IRIS program; establishing clear expectations and responsibilities for all parties involved; assessing for participant risk; ensuring a smooth transition to the IRIS Consultant (IC); and assisting in the development of the initial Individual Support and Services Plan. In addition, the Orientation Consultant works collaboratively with the regional Consultant Mentor to provide ongoing planning support, expertise and resource to participants, Mentors, and Consultants as requested.
Responsibilities: Collaborate with participants to implement their initial Individual Supports and Services Plan and as needed, assist in continual implementation; provide Welcome Call to the person referred and schedule the initial face to face meeting; provide a thorough program orientation during the face to face meeting; complete the Initial Inventory which assists in providing a broad view of the participant's life; engage in conversation with the individual and his/her support people regarding responsible use of scarce public funds and creativity in planning; verification of legal documentation – Power of Attorney, Guardianship, Durable Power of Attorney for Health Care; document all referral activities in the IRIS data system; complete Allocation Adjustment/Exceptional Expense paperwork and collaborate with AA/EE Specialist as required for securing a start date when needed; involve additional planning supports available through the ICA or support network as needed; Guide participant and family in choosing an IC; ensure a smooth transition to IC by completing all the required paperwork, discussing the participant's plan and outcomes with the IC, and attending the IC'S first visit with the participant; When requested, provide planning assistance to IC'S in engaging participants and their support circles in a more intensive planning process; document this planning process in the Enterprise data system; develop clear action plan and identify specific tasks that the Consultant can assist the participant in achieving; perform initial review of participant health and safety issues that may be relevant; document health and safety issues and develop a plan with the participant for addressing or mitigating risk; discuss health and safety risks/concerns with IC Mentor and IC; Involve additional supports as needed to ensure health and safety; Communicate any immediate health and safety issue the appropriate authorities; Provide orientation to IRIS Self Directed Personal Care Program (SDPC); Review individual's eligibility for SDPC with the participant and/or participant power of attorney; Request individual be added to referral list if interested in the program and found to be eligible; Communicate SCPC requirements to participant and/or participant power of attorney; Assist ICA and participants in ensuring and maintaining participant financial eligibility; perform room and board calculation for individuals living in substitute care as requested; meet weekly with area Mentor to collaborate, plan and problem-solve; Provide time and expertise to IC's, Mentors, and program participants as requested; participate in discussions and problem solving regarding overall program development and implementation; and assist with projects as requested.

Qualifications: Bachelor of Arts or Science degree in health, human services or a related field; must be a certified Adult Long-term Care Functional Screener and maintain certification as a screener; at least one year experience conducting the Adult Long-term Care Functional Screen and working with at least one of the three target groups served by IRIS; strong writing skills and attention to detail; strong computer skills using various software applications; strong customer service skills; good organizational and time management skills; the ability to work cooperatively as part of a team; the ability to work autonomously; must possess a valid driver's license, maintain adequate auto insurance for job-related travel, and ability to travel within Wisconsin and other states as necessary; flexibility in the work environment and willingness and ability to adapt to changing organizational needs; and a commitment to the mission and vision of the IRIS program.

Service Center Operations Lead
The Management Group, Inc. is seeking a Service Center Operations Lead who is responsible for coordinating the activities of a variety of the IRIS Service Center's sub-functions. This includes providing day-to-day guidance for the employees engaged in program enrollment, partner relationships, financial eligibility, and clerical support. This position's responsibilities include solving complex issues, tracking information and progress, coordinating workflow, training on new programs or procedures and generally facilitating effective processes for the team.
Responsibilities: Develop and coordinate weekly staff schedules to ensure minimum staffing; work with supervisor on workplan development, goals and tracking; provide guidance and feedback for workplan management; make recommendations on quality improvement opportunities to IRIS management; follow up and resolve issues with the input from IRIS management when appropriate; participate in the development and implementation of functional operating guidelines; participate in the development and revision policy and operational guidelines as appropriate; update information/resource materials for new employees; develop and manage training materials; train new employees; provide and/or facilitate training/development and orientation opportunities to ensure staff have tools and knowledge to be effective; provide ongoing mentoring to staff; ensure staff timesheets are reviewed for accuracy and timeliness; act as an effective resource to staff, participants and stakeholders; partner with various internal and external resources to ensure effective operations; provide program-related support to other staff; liaison with program partners and stakeholders on operational issues and projects; assist with development of work plans for subcontracted work projects and ensuring completion of the work outlined in these work plans; represent IRIS in workgroups and committees as requested; explore and communicate opportunities for IRIS involvement in activities and initiatives being undertaken by the stakeholder community; take an active role in these networks; analyze data and inform/report findings to appropriate parties; track timelines and milestones as documented; review reports to ensure plans are being processed in a timely and efficient manner; provide support and assistant to IRIS quality management team when requested.
Qualifications: Associate's degree or commensurate experience required, a Bachelor's degree is highly preferred; two years of applicable work experience, including operational responsibilities and supervisory experience; individuals with solid experience in and strong knowledge of Wisconsin's long-term care programs and familiarity with principles of self-determination are highly desired; basic math skills; strong interpersonal and customer service skills to build relationships with a diverse population, managing a variety of personal situations and issues; good organizational, time management and prioritization skills; strong communication skills, including written, verbal and listening, and the ability to adapt communication style to fit the situation and audience; strong mentoring skills; strong analytical and problem-solving skills; ability to be flexible, patient, creative and innovative in day-to-day operations and situations; ability to operate in an open, honest and forthright manner in order to build and maintain relationships; ability to focus on multiple priorities simultaneously; ability to be resourceful, proactive, detail-oriented and learn and incorporate new items quickly; ability to be responsive, empathetic, customer-focused and quality-driven; ability to remain calm and rational in high-pressure situations; ability to operate in a professional manner as a team leader; ability to be self-motivated and patient; ability to work both collaboratively and independently at a fast pace without close supervision; ability to work cooperatively as part of a team; ability to be both persuasive and authoritative, depending upon the situation; ability to problem solve, assessing situations quickly and effectively using resources available and selecting a course of action and moving toward it; ability to analyze data, draw appropriate conclusions and implement solutions; ability to problem solve, troubleshoot and follow through/persevere as needed and appropriate; ability to travel as necessary; must possess a valid Wisconsin driver's license and proof of personal auto liability insurance meeting Wisconsin's minimum requirements; flexibility in the work environment and willingness and ability to adapt to changing organizational needs; demonstrated computer and software skills required; and proficiency with Microsoft Office Suite and database operation/maintenance skills and data entry experience.
