As of April 30^th, 2013, Deaconess Home Health is no longer available to provide Medical Assistance Personal Care and Supportive Home Care in the Southeast Area. The IRIS Consultant Agency has worked to create resources and support plans to strengthen transitions for those impacted by this change.  We encourage you to reach out to your IRIS Consultant if you are in need of supports and would like to access these developed resources.
